How to Choose Between Submuscular and Subglandular Placement
Subglandular and submuscular breast implant placements each give a uniquely different flavor to the final result. Each approach also has a different long-term effect.
Subglandularly placed breast implants are positioned in contact with the existing breast. They provide sharper cleavage and better fill the bra cup. However, since the weight of an implant adds directly to the overall breast weight, there is a greater possibility of sagging with the subglandular approach. In addition, the rippling of a breast implant is more evident when inserted subglandularly, because it is closer to the skin as compared to the submuscular approach.

Submuscular breast implants are inserted behind the muscle and are not in direct contact with the existing breast tissue. A breast implant, in this case, will simply move the existing breast forward by pushing the muscle layer into the breast. The breast weight is somewhat supported by the muscle. For this reason, there is less long-term sagging.
