Breasts that are disproportionate to the body because of their size can be reduced to restore more natural proportions. The degree a breast is reduced varies from a minimal reduction to an extreme reduction – based on its starting point, the patient’s desire, and the surgeon’s aesthetic judgment.
Breast reductions fall into two categories, Cosmetic Breast Reductions or Functional Breast Reductions.
Cosmetic Breast Reductions:
In this case, breasts are not too large or heavy to cause any health issues. However, they are out of proportion with the body. A subtle to moderate-size reduction is all that is needed to create a harmonious bodily appearance. Surgery in this case is strictly cosmetic in its goal.
Functional Breast Reductions:
This subgroup of large breasts is large enough to cause symptoms. Symptoms may be back pain, shoulder strain, and pain, even shoulder grooves from the pull-on bra straps. Sometimes, but not always, large reductions may be covered by some but not all insurances. Usually, authorization from the insurance company is required before proceeding with surgery.
Surgical Technique:
Many different techniques are at disposal of plastic surgeons for breast reductions. A surgeon will use a combination of available surgical techniques along with his/her take on those to deliver the best possible outcome. Good surgeons do not depend on a cookie-cutter approach. Markings for new design
The new nipple-areola position is determined first. This is determined by using multiple reference points on the body that are then, blended with the aesthetic sense of the surgeon. Locations of breast creases, collar bones, and the midline of the chest and arms are used as reference points in determining desired nipple-areola position.
Since large breasts are long, the amount of tissue needed to shorten the breast is marked. Since large breasts are wide, the amount of tissue needed to narrow the breast is marked. Since large breasts extend to the patient’s armpits, this excess is marked for reduction or liposuction. Many times, liposuction will be needed around the new breast mounds to define their shape. This is marked as well.
Anesthesia
Breast reduction is done under general anesthesia. Only an anesthesiologist who is certified by The American Board of Anesthesiology gives anesthesia in our Cosmetic Surgical Center.
Surgery
The surgery starts by repositioning the nipple-areola complex to its new position. Most of the time it is accomplished by using bridges of deeper layers of skin that carry the attached nipple-areolas to their new location and still preserve most of their connection to the underlying breasts. These bridges are called pedicles. Several pedicle designs are available. Namely, superior pedicle, inferior pedicle, oblique pedicle, etc.
If the breasts are very large or the nipple-areola complex is too low a Free Nipple-Areola Graft technique is needed since the pedicle techniques will not provide an adequate blood supply.
Patients who can be reduced with pedicle techniques sometimes can breastfeed. This function is lost with a free graft technique.
In the next step, the vertical and horizontal skin excess is removed.
The breast size is reduced. The shape of the new breast is enhanced as it is reduced. Essentially, it is a four-dimensional breast shaping.
Sometimes drains are used. Sometimes drains are not necessary.
Recovery
Breast reduction is an outpatient surgery. Pain is mild to moderate. Drains can be removed in 2-3 days. Most stitches are removed in 7-14 days. Full activity is permitted for six weeks.
Risks
All surgeries have some risks. With breast reduction surgery, there is a risk of scarring, circulation concerns, bleeding, loss of skin, loss of nipple-areola complexes, the risk to breastfeeding capability, and loss of sensation.
Achieving aesthetic goals in cosmetic breast reduction and relief of painful symptoms in cases of a functional breast reduction usually outweigh any negatives.
